The second R-rated movie that I saw (in the theater, as home video wasn't a thing yet) was Two Minute Warning; the random gun violence of it was much more terrifying to me than The Exorcist had been. Once I hit high school, nothing was off limits.

I can't say for certain it was the first one, but I recall Friday the 13th, Part V: A New Beginning in the theater. I would have been 12 at the time. I'm not sure what my dad was thinking. I had seen the earlier ones on cable so I must have begged to go.

Probably Get Shorty at 10. Looking back, it's a random one for that distinction. I first saw RoboCop at 6 on VHS, so it's not like this movie was anything too crazy.

My parents took to me to Drive-In double feature of Mario Bava's Beyond the Door II and Romero's Dawn of the Dead sometime in the late 70s (when I was 10). Oddly enough I remember pleading to see Halloween around the same time but not being allowed (presumably because it had boobs, which is much worse than zombies ripping out and eating a guy's intestines
).

National Lampoon's Vacation when I was seven or eight. The next one was John Carpenter's adaption of Stephen King's Christine.
Wasn't sure which one I saw first. I had to look up the release dates.

Any popular rated-R prior to 1982? Just checked, aside from anything before 1981, I’d say I believe I saw Escape From NY from 1981. I think. But, definitely saw Road Warrior, I’m sure with Dad in 81. It’s a blur, but I do believe.

Same, but it was at a drive-in when I was 5. I don't remember a thing about it.
As for in the theater, I don't really remember so I took a look at some movies lists for the late 70s/early 80s, and it might have been Young Doctors in Love. There were a few R-Rated movies I saw around that time though.

The Good Son. I was 14 and bought the ticket with no issue. Not nearly as good of a movie as most of you, but I remember the excitement of seeing a R rated movie in the theater by myself.

I can't recall with 100% certainty, but I think it may have been Silence of the Lambs in 1991. I went with a high school buddy. I couldn't go to very many movies as a kid in the 80s because I had no money and my Mom had to make time to see movies with me and that was very rare,

I believe the first one I saw with adults was Jackass the Movie, and I know the first one I saw alone was Silent Hill it came out the day after my 18th birthday.

Depending on how you look at it,, it would be Bonnie and Clyde instead. The movie predates ratings, but I saw the 1969 reissue (age 11) in which it had been submitted for rating and received an R. I think it's still rated R.
Saw Scars of Dracula/Horror of Frankenstein double feature at drive-in 1970. Predates Dirty Harry.
Worthy of note: saw Blood Feast in 1963 at age 5.

I saw a double bill of 100 RIFLES and COOGAN'S BLUFF in 1969 when I was 15. 100 RIFLES was rated R. (I'm not sure why, it sure looks PG today.) COOGAN'S BLUFF had been released before the ratings were instituted and was tagged on posters only as "Suggested for Mature Audiences." IMDB now lists its rating as R, but it wasn't identified that way back when I saw this double bill. I still have my handwritten viewing log from that year and it says "R" for 100 RIFLES and "SMA" for COOGAN'S BLUFF. Other R-rated movies I saw that summer were THEY CAME TO ROB LAS VEGAS, THE WILD BUNCH, THE BIG BOUNCE and GOODBYE COLUMBUS. I attended all of these movies alone.

THE EXORCIST with an older friend. The experience was grand. First one I saw by myself was THE AMITYVILLE HORROR. First and only X-rated film in theatre was CALIGULA, which was most abominable, among the worst films I've ever seen.
