The Screen Gems "S From Hell"--Who knew there were millions of other kids besides me scared of that stupid logo? I seriously thought I was the only one for many years.

There was another one from about 15 years ago that gave me the creeps, but I can't remember it exactly. It seems like it was for Pizza Rolls or Hot Pockets or maybe even Domino's, but I couldn't find it on YouTube without more details. It had a mother and 10-or-so year old kid sitting at a kitchen table. It was a fairly bland commercial - there were no special effects, but some marketing person must have decided to do some retouching in post production. The kid's eyes were CGI'ed or something to make them look larger and the whites were really white. It ended up looking very bizarre. I know that's not much to go on. Anyone else remember that one?
Edit: This isn't it, but it was similar. This one is scary too.

I remember an anti-drug commercial from at least 35 years ago (when I was a kid) that talked about kids "nodding off". I was horrified by it and didn't really know what it meant, but it sounded terrible.

I was actually a teenager when these aired, but they freaked me out.
Back before The Ring came out, there were these commercials for it that aired between 1 and 4 AM. They would start with a couple of seconds of static, would show some footage of the cursed tape, then cut to the shot of the well and then back to static. All while some creepy voice said "Before you die, you see the ring" over and over.
Mind you that this was before I had heard of the movie (months before any other advertising for the movie started), they were late at night at random times, and had no credits, title, coming soon card or anything (Simply the footage of the cursed tape). Over the course of a few weeks, the commercials got longer, with more tape footage. Eventually they got to the point where you would see the hands reaching out of the well before it cut to static at the end. Still, they never identified what they were advertising.
Closer to the opening of the movie, they tacked on the title and some footage of Naomi Watts...but damn those original commercials freaked me out at the time. It's too bad I can't find them on youtube.

Had avoided the film until the DVD release, and watched it for the first time, alone in the middle of the night. I lose myself into films, and can get gripped by fear by even the silliest horror films. Personally, I love this film and find it the rare bird where the American remake is better than the original, so I was scared silly all film long pretty much. Scared to get out of bed or look almost anywhere for fear of seeing "her", I clicked around the special features after the movie ended. I found an easter eggish place that said, "Don't click here."
So of course I clicked. It immediately launches into "the video". The fear I had at that moment was the most intense of my life. Of course I knew deep down that it was all manufactured fear, and that no girl from a well was really going to kill me in 7 days, but at that brief moment I was so mortally terrified that tears were produced. Only time in my life where that has happened. Sort of wimpy I admit, but it was so intense I almost want to be able to recreate it.

That It's Alive trailer had a link to this one for Beyond the Door that used to scare me and my brother when it played late at night on TV. Finally saw the movie in real life years later and it was pretty dull. All the good stuff is in the trailer.

The Ring did scare the crap out of me, I'm not ashamed to say - and I was 17 when it came out. I went to a matinee at an old rundown theater on a rainy weekday. I'm sure the guy selling the tickets/running the projector was the only other person in the building. I saw it again with a packed house and it didn't have nearly the same impact, but I didn't sleep comfortably for next couple nights - especially since my TV was facing the bed.

But more on topic, I always found the dancing old dude in the Six Flags commercials a little unsettling for some reason....

I always thought the music in these commercials was creepy. Funny thing is that they were brought back for a while in 1987, when they already looked ancient.

I love that the PSA also has a really good message, even though it scared you as a kid!
A scary commercial for me: advertisements for Fire in the Sky, especially with that membrane thing over him. Still freaks me out!

I was scared to death of Rocky Dennis as a kid. My mother was watching this old show called PM Magazine when the movie was in theaters and they were showing pictures of the actual kid. It freaked me the F out. By time Mask started airing on TV, it was around 1988 and I was around 12. Was watching some late night TV with my mom one night and BOOM! Rockys face is on the screen "Mask, Friday night @ 9pm". Needless to say, I didnt sleep that night and closed my eyes every time commercials ran on that channel for a month. When I was around 17-18 I decided to overcome my fear and rent it.
