Alternative reality is linked to the idea of parallel universes, which is a science fiction conceit for story-telling.

Since Friends is not a sci-fi show, the premise of this episode is not that "what could have been" is a representation of another reality that actually exists in another dimension or alternative timeline, the premise is that these are just the imaginings of the characters made manifest for the viewers to observe.

This story-telling conceit is obviously very common in sit-coms as "dream" episodes. The twist here is that all of the six friends are dreaming together.
